Pet Adoption Questionnaire

Thank you for your interest in adopting a pet from Whitby Animal Services. This questionnaire is designed to provide you with a few questions to think about before you adopt. In order to be considered for an adoption from Whitby Animal Services you must be 18 years of age, and provide valid identification including your current address.

Terms and Conditions of Adoption

1. I agree to treat the animal I am adopting humanely. I will educate myself with respect to my responsibilities towards the pet that I am adopting. I agree to comply with the Cat and DogBy-law.
2. I agree that I shall provide appropriate care, adequate food, water, shelter, exercise and attention to meet the needs of my new pet.
3. I understand that Whitby Animal Services does not have a full time veterinarian on staff and that animals may have only received veterinary care if required. Although every precaution is taken to ensure that they are in good health, I understand that the animal I am adopting may be in need of additional medical attention. I further understand that due to the numerous animals housed at Whitby Animal Services, animals for adoption may have been exposed to transmissible diseases or parasites. This may include respiratory infections, internal parasites, fleas and/or ear mites. The symptoms to watch for are:
   - Coughing                          - Diarrhea
   - Lethargy/Depression        - Sneezing
   - Vomiting                            - Fever
   - Discharge from eyes/nose or mouth
   As the new owner, any costs incurred will be my responsibility. I agree, Whitby Animal Services will not be responsible for reimbursing any costs incurred by me for veterinary treatment.
4. I understand that Whitby Animal Services recommends taking the animal I have adopted to a veterinarian for a complete health examination. If an adopted animal is examined by a veterinarian within 72 hours of adoption and the animal is found to have a serious medical condition, I may return it to Whitby Animal Services for a refund (with documentation from a veterinarian) withinone week of adoption, or choose to treat it at my own expense.
5. Whitby Animal Services provides basic vaccination for adopted cats and dogs including a one-year rabies vaccination (rabies vaccination is not provided for kittens under 4 months as vaccination at this young age is not possible). I agree and understand that any specialized or additional vaccinations (if necessary) which may be recommend by a veterinarian are my responsibility.
6. Staff at Whitby Animal Services make their best efforts to evaluate the temperament and behavior of each animal made available for adoption. However, despite these efforts, on occasion an animals temperament/behaviour may change when exposed to a new environment. I understand thatWhitby Animal Services makes no claims, guarantees or representations as to the temperament or disposition of any animal, and additional training may be required at my own expense.
7. I understand that this animal will affect all aspects of my life such as, home, travel, family, and other pets.
8. I agree that by adopting this animal from Whitby Animal Services that I am now fully responsible for ownership of this animal.
